In preparation for your appointment: 

Please call the member services number on the back of your insurance card to ensure your policy is in-network with SkinDC. We are not always able to confirm what type of policy you have by looking at your card. It is important that you confirm with your insurance that SkinDC (or the provider you are scheduled with) is in-network with your insurance policy to ensure your visit will be covered. 

For example, even though we accept BCBS, your policy may not be in-network with SkinDC and you may be subject to higher fees or not covered at all. We highly recommend you determine this information prior to your appointment since we will not have confirmation until after we submit your claim.

If you have an HMO insurance policy or any other health plan that requires referrals you will need to provide a referral from your primary care physician so that insurance covers your visit. Some insurances require referrals for each office visit and also for procedures. Please bring this to your appointment or have it faxed to us prior to your appointment. Fax. 1-844-357-7049 

Failure to present a referral at the time of your visit will require you to reschedule your appointment.

If you prefer not to reschedule, you will have the option to be a “Self Pay” patient and submit the claim on your own. 


Exclusions: 

You must confirm with your insurance prior to your appointment if you have exclusions on your insurance policy. For example, you confirmed with your insurance you are in-network with SkinDC, but you may only be able to be seen for acne because all other concerns are excluded from your policy. We are not able to determine what your exclusions are by looking at your insurance card. Please call the member services number on the back of your card to determine details. 

Please bring your ID and Insurance card with you to your visit.

Self Pay: 

If SkinDC does not accept your insurance, we are still happy to see you as a Self Pay patient. Cost is determined based on the extent of your visit. Most office consultations are $150-$250. Any treatment provided would be an additional cost and pricing would be provided upon evaluation.
